Cards are not added on mac without any error.
The Notification issue #1 is also on mac so error might be there but notification might be blocked.

Steps:
Test on Mac somehow to reproduce the error.

Suggestions by NikBike:

Provide settings to store(default) or retrieve
deckNames, modelNames, and fields for each note once.
This will definitely decrease load time which is ~10 seconds on macOs for some users.

tested in macOs Anki 2.0 & 2.1 ankiConnect-5, ankiConnect-6 Chrome (66.0.3359.181)

Probably the requests to AnkiConnect is happening every time when I open extension to look into to check my notes of fields. And as result, it takes about 10 seconds every time. It'll be faster to switch to Anki application - this program has a convenient editor and many add-ons for formatting and hotkeys (IMHO)

I couldn't add the card, in my case. Just not added without any errors. I tried by submitting via context menu and via the menu of extension.

I'm not quite following your goal of this development, but Thanks anyway for sharing.
