Include crash counter and suspected mods in crash report

• Improve world load speed by not showing the "Downloading terrain" GUI (almost as fast as same dimension teleport now!)
• Enable GUIs in nether portal
• Remove vanillafixcore from mod list
• Fix sponge compatibility
• Fix duplicate mod with latest forge
• Fix integrated server crashed not being catched
• Fix race condition when crashing in singleplayer
• Clear scheduled task queue after client-side crash to avoid error spam trying to handle packets
• Crash on exception in scheduled task (will be configurable)
• Fix nether portals placing the player at the same position in the nether
• Alt + F3 + C to trigger a test crash on the integrated server
