To quote from Discord (Beesechurger#9215):

In a 5% chance, if the bolt is Arrow (nbt should show), the loaded arrow is turned to Duration 1 Blast firework, thus dealing area damage

Possible Item: Enchanted Dispenser (15 XP levels)
