Not sure if we'd call this a bug or not, but it's a really annoying quirk. I ran into this during my most recent Madness seed, you can see it happen a few times during this VOD on Twitch: https://www.twitch.tv/videos/2366322708
Basically, with those settings you usually end up needing to do Bottle OI a lot to play an Ocarina, even more so than usual because of Fairysanity. So for the Gossip Stones, I would play one song using Bottle OI, and if the item that appeared was Junk, I would just ignore it because sometimes the fairies are annoying to pick up. I would drop the Bugs/Fish again to pick them up to do OI for the other song, but sometimes I would actually pick up the "Fairy" that spawned from the first song. This would fill my bottle with a fairy, which effectively meant that I lost the original bugs/fish. I would still be able to OI with the fairy, but only once, since I wouldn't be able to drop it and re-pick it up again later. In settings like Madness this is a massive inconvenience due to the decoupled entrances, it's usually not simple to just "find the nearest bug patch".
Proposed fixes:
Swinging a bottle at the "fairy" check will cause you to collect the item instead of filling your bottle, like how Fishsanity checks work. This might make the check easier to obtain and prevent filling the bottle so I can still pick up the original bug/fish. I would imagine this fix also applies to the other song fairies and the fairy fountain fairies pretty easily.
Instead of spawning a Fairy when playing the song, just give the item immediately. This looks less cool than having the check fly around, but picking up these gossip stone fairies is really annoying lol.

my vote is option 2. As cute as it is to have the items flying around, you still have those in the fairy fountains and when you've got to do this dozens of times in a run, chasing the items gets old very fast.
