OGC-25 QR style barcode

Create an alternate barcode format to use a QR style barcode format.
This is how the layout would look on the current default barcode size, this should scale down fairly well too.

The barcode itself is the lab number (same as the large text) without dashes

There should be a dropdown select on this page: OpenELIS
At the very top, the label should say Barcode Format: and have 2 options, Classic and QR.

The default should be classic.
