Pinned Outlook add-in icon not showing

The icon of a pinned Outlook add-in is not showing in OWA in the ribbon. The name of the add-in shows, but no icon is loaded. This is an add-in that is side-loaded or admin-managed deployed using a manifest (not deployed to the AppSource Store). This is seen when pinned in Compose mode.

This has worked fine in the past. Not exactly sure when it stopped showing up, but we know 100% it worked previously and now it is being reported by customers that the icon is not being shown.

Your Environment

• Platform [PC desktop, Mac, iOS, Office on the web]: OWA
• Host [Excel, Word, PowerPoint, etc.]: Outlook
• Browser (if using Office on the web): Chrome

Expected behavior

The icon is shown for all add-ins, whether or not they are deployed to AppSource (if that's the issue).

Current behavior

The icon has disappeared fairly recently and no longer shows.

When in Read mode, the add-in does show in the toolbar on the message with the correct icon:
