Add "nameplateHideRealmNames" cvar for quality of life

Other cvars is appreciated but not required: "nameplateTextSize", "nameplateTextOutline".

Absolutely agree, especially with nameplateHideRealmNames. There's no un-linked server on TW region retail server.

There is currently a problem. when "nameplateShowOnlyNames" is set to 1, the game does not adjust the position of the elite icon and raid target icon. They still appear where they are supposed to be (to the left of the health bar), even if the health bar is not visible. this makes it difficult to visually see the location of the raid target
Especially in the instance where friendly nameplate modification is disabled
