install.sh patches FFmpeg sources, but install instructions don't include this #22

In the Install Instructions section of the manual, it says that performing the listed steps manually does the same thing as running the install.sh script, but that's not true: the install.sh script also patches the FFmpeg sources.

I have fixed the manual to use the correct links and discuss the required patch.

How do I run doxygen to see how it renders the manual? It looks like there is a config.cfg file that is very site-dependent on Diamond?

I don't know. I've been making changes to it blind (i.e., without actually running Doxygen).

The doxygen config file is a leftover from before we published to github, and before we'd discovered markdown or restructuredText. @ulrikpedersen might know what happened to the docs build when we moved to github...
Personally I don't think the doxygen autogenerated docs are worth the hassle any more, so I'd be happy to just have the docs in markdown or restructured text...
