VBA Facility service header & Facility Service Descriptions appear on some Vet Center - Facility Services and VAMC System Health Services

When adding a service to Vet Centers or VAMC System Health Service that is also available to VBA, the "Facility service header" & "Facility service description" shown is from the VBA version of the service and not the Vet Center version.

• The "General service description" for the following content types should not show "Facility service header" or "Facility service description," which are particular to VBA Facility Services:

@Agile6MSkinner @jilladams In the course of working on this, I discovered that the VBA Facility Service-specific fields were also showing on the VAMC System Health Service descriptions of services that are also used by VBA Facility Services. I updated the description to reflect that.
