Nested lists are flattened

Describe the bug

I'm not sure how widespread this is, but at least in this example, nested lists are flattened into a single level:

Expected behavior

Nesting should be preserved. For example, there should be three list items in a nested list in the first item (see the AsciiDoc screenshot).

Steps to reproduce

1. Build the elasticsearch.md doc set
2. Go to http://localhost:5000/remote-clusters-api-key.html#remote-clusters-security-api-key-remote-action
