QBX order greater than FMM order
Stage 1 order greater not equal to QBX order
Stage 2 quad order less than twice QBX order ("too low")?

@mattwala, can you think of any others?

TCF is too large (causes the target confinement region to exceed the near-neighborhood size)
expansions_in_tree_have_extent is set to false
