mutex_unlock(&av->ip.csi2->stream_mutex);
if (av->ip.csi2->stream_count == 0)
entity->pads[0].pipe = NULL;
mutex_unlock(&av->ip.csi2->stream_mutex);

I think the first unlock should be lock ... the code review didn't capture this.
