The task is not attaching to every edit

Every time I toggle the check box, there is a new action that is queued, but the Task Completed Checker is only attached to first
time the action runs. For every subsequent run, the task-check runs, that edits the status of Task Completed Checker in first run, but how do I make it required, because in subsequent runs, I dont see Task Completed Checker and hence the checks pass even if checklist is not ticked.

The required check is waiting for reporting, but the task-check reports it to first run only and not to current run.
