Specification states it is mandatory to have an end date, but this conflicts with the profile where it is not mandatory and the specification that it will match medStatement effective date which does not have a mandatory end date.

Change the spec to align with profile / med statement / initial implementations.

May need to be changed in the future subject to #1101
