Extend top nav bar design for ODK-X Services to ODK-X Survey app

The ODK-X Survey mobile app has a mix of icons and text items under the ellipsis menu which do not communicate the function they are linked to. However, the design for the top nav bar has been updated on the services app to meet the ODK-X Design Guidelines.

By extending the style implemented in the ODK-X services new app to the Survey app, we make the menus consistent across the all applications.
