USB timeout

We hadn't seen the yubihsm software stack timeout when creating an 4k RSA key since March when we bumped the timeout up to 5 minutes ... until we did. We need to do a root cause to see if the timeout configured at the connector is to blame or if this is coming from somewhere else. Ideally we'd be able to recover if possible.
