Provisioning process: need a simple page and seed script

We need a simple web-form to enroll patients that gets the PIN and Start-Date. The PIN in this case is tied to a phone, and if need be we can pre-provision these PINs (whatever is easiest). Chintan has a Python script to populate user activity instances once enrolled, which we can use or we can rewrite within the node portal. Whatever is easiest and stable is fine here.
