Improve performance in CaseContactsController#index

Consider adding tests using something like https://github.com/makandra/query_diet or https://github.com/julienbourdeau/debugbar or https://github.com/rubysamurai/query_count or similar to find and improve performance issues. Consider rewriting how we do pagination if that helps :)
