upload-notes should check for other kinds of FHIR resource names

This is more about the fhir spec than anything else - since patient_id and subject_id are used interchangably, and docref_id and documentreference_id are as well, upload_notes should check for a set of potentially valid labels rather than an explicit one.

Yeah, fair. And we should also probably accept _ref versions of both and strip out Patient/ or DocumentReference/ as necessary.
