Cumulus Library 4.0 release

You can install the latest library release with pip install -U cumulus-library. The release notes are over on the project github.

This release adds support for the FHIR AllergyIntolerance resource. It also introduces a new workflow (what we're now calling the TOML-driven builders) for generating valuesets from a subset of codes/keywords. Manifest formats have been updated to allow for more customizable import/export formats.

The important breaking change in this release is the removal of a floor date value of 2016-01-01 in the core study (which was really a limit from one of our first cumulus studies). If you have been relying on this behavior, or have an in-progress study effort, note that this change may affect your datasets. It is recommended that each study define its own minimum/maximum date ranges as part of its patient selection logic.
