PHP Fatal error in SpecialPage.php

What steps will reproduce the problem?
1. Adding require_once("$IP/extensions/Yubikey/Yubikey.php"); LocalSettings.php


What is the expected output? What do you see instead?

I should be able to use Yubikey to authenticate, but instead get a blank page 
with the following error in apache logs:
