more flexible commands, dmenu improvements

• cmd_to_script/cmd_alt_to_script: will create a bash script in /tmp that will be exucted
• applications module will now always create all entries and not skip the general one if actions exist
• various new flags introduced to improve usage with dmenu-mode: --placeholder, --labelcolumn, --keepsort
