This makes `CacheKeyLock` a trait that the current whole-file level implementation `CacheLock` implements. The visibility changes are a result of making the trait public for re-implementation.
This adds an extensions public member to the CacheKey so that arbitrary extra data can be attached via the `cache_key_callback`.
