Hello, Enterprise World

Overview

This is a not-so-basic Hello World App, covering

Express 4.16
https://helmetjs.github.io/ for security
tests (mocha, chai) and test coverage (nyc)
Log4js for JSON formatted log output.
Correlation IDs in all logs, and on all outbound requests.
