Modifiers
You can create, edit or delete Modifiers in the Modifier Manager. You can open the Modifier Manager by clicking here:

The basic overview works the same as the Table Mode of binding a relation. Click on add Modifier to add a modifier. Once you click on it, you have 10 Seconds to click on a DirectInput button or Keyboard button.
Once you clicked it, a new line will be displayed. The Mod Name is the Name it will show for this modifier in the Dropdown menu and also within DCS. The next column is a button to delete that Modifier. The next column it shows the device which made the Input. The following one shows the Button that is the Modifier. The last column lets you decide if it should work in switch mode.
Once you are happy just click close. Before you can assign Modifiers to a relation, you need to set the basic button press to it, to which it should modify. Once that done the Dropdown selection of modifiers will work.
